Our Memorandum of Understanding Collaborative Partners
We like to say SASH is a team sport. That’s because it’s a carefully organized model of care which relies on a vast network of partners throughout Vermont. These partners work together to help older adults and people with special needs who are receiving Medicare, receive the support they need to remain living safely at home.
Our partners are key members of local SASH teams who coordinate the care and services for participants to age in place and receive the care they need to live safely and independently at home.
