SASH Coordinator
Put your social-work, organizational and management skills to work helping Vermonters make healthy decisions and remain living at home as they age. This is a position with lots of variety and lots of action.
You’ll form trusting relationships with residents, help develop individual and group health plans, coordinate regular activities and events, record and maintain confidential health information, and interact regularly with aging-services and health-care providers, family members, SASH teammates and others.
You’ll also create a monthly resident newsletter, organize and facilitate monthly resident meetings, and serve as the key on-site contact for issues relating to housekeeping, noise, site disturbances, lease violations, etc.

SASH Wellness Nurse
As a SASH Wellness Nurse, you’ll work with participants in one or more SASH panels (groups of up to 100 participants) and support them in reaching their personal health goals. You’ll conduct health assessments, help participants manage chronic conditions, and get to know participants on a personal level.
You’ll also collaborate with the SASH Coordinator to develop programming and workshops for the group, convene meetings with local SASH partners to coordinate care, and provide individual support as needed. And you will enjoy a lot of variety, camaraderie and appreciation for your good work.

Cathedral Square
SASH Emotional Wellness Clinician
The SASH Emotional Wellness Clinician (SEWC) provides low barrier, direct therapeutic support in the home including supportive counseling, individual and depending on training, couples counseling, and facilitation of group programs aimed at promoting emotional wellness. The SEWC also provides screening/triage services, care coordination, consultation, staff education, and referrals. These services are offered at no-cost to all SASH participants living in the identified SASH sites, based in affordable, congregate housing. The SEWC is part of the SASH team, comprised of a Wellness nurse and SASH coordinator, who work together to support older adults and individuals with disabilities. All SEWCs are currently hired, contracted and supervised through participating Designated Agencies and integrate into the SASH teams.
